Personal Care

The HealthKeeperz Personal Care Services is a fundamental program to the HealthKeeperz continuum of community based health care services. The HealthKeeperz Personal Care Services (PCS) helps many people maintain their health, independence and virtue in the comfort of their own home. HealthKeeperz brand of personal care service is like no other. We strive to provide the right care, in the right place at the right time. If you or a loved one is having limitations with common activities of daily living such as bathing, dressing, accessing a restroom, eating or personal grooming, contact us so that together we can achieve great things!

Hospice Care

HealthKeeperz Hospice division provides care and services that combine, state of the art medical care with emotional and spiritual support for our clients and their families. HealthKeeperz Hospice focuses on palliative care when curative treatments are no longer desired. Ultimately, HealthKeeperz Hospice seeks to provide physical, emotional and spiritual care when the perspective of life shifts from quantity to quality. The HealthKeeperz Hospice team includes nurses, pastors, social workers, volunteers, and in-home aides.

CAP Case Management

CAP (Community Alternatives Program) is a program sponsored by the State of North Carolina as an alternative to nursing home placement. A patient must meet the eligibility criteria for a skilled nursing facility and qualify for North Carolina Medicaid. HealthKeeperz provides CAP Case Management Services for both Adults and Children. Once on the CAP Program a person can receive additional benefits which they cannot receive through traditional Medicaid. CAP exists to supplement, rather than replace, formal and informal services and family support already available to an individual. HealthKeeperz understands that people are happier and healthier in the comfort of their own home. Likewise, HealthKeeperz believes patients are able to maintain the highest level of independence and virtue in the comfort of their homes.

Home Medical Equipment

HealthKeeperz Home Medical Equipment (HME) focuses on helping individuals maintain their independence, health and virtue in the home. The HealthKeeperz Department of Home Medical Equipment provides the products and supplies that help individuals stay healthy in their home as long as possible. HealthSync Pharmacy

HealthKeeperz HealthSync Pharmacy Program synchronizes your medication to be delivered on one day each month, eliminating multiple trips to the pharmacy. In addition to the free delivery, you receive:

• A HealthKeeperz PocketCard containing emergency and physician contacts, current prescription information and known allergies. This card is compact to fit in your wallet and carry with you in the event of an emergency.
• Healthy Choices Program allowing you to choose a FREE item with your monthly delivery. These free items, such as rubbing alcohol, band-aids, vitamins, and more are our way of helping your stay a little healthier and saying thank you.
• HealthSync Medication Report, a summary of your current medications and prescription history will be sent to your doctor every three months to assist him/her in their efforts to stay current and provide you with the best care possible.
