A Message from Our CAP Director

Every day, I meet adults and families who want the same simple thing:
to remain safely in their own homes, surrounded by familiarity, dignity, and independence.

What often stands in their way isn’t a lack of desire, it’s the complexity of the healthcare system.

As Director of Community Alternatives Program (CAP) Case Management at Healthkeeperz, I see firsthand how overwhelming it can be to navigate Medicaid services, understand eligibility, and coordinate the support needed to live safely at home. Many families don’t even realize help exists until they are already in crisis.

That’s where CAP Case Management comes in, and where Healthkeeperz walks alongside you.

What Is CAP Case Management?

The Community Alternatives Program (CAP) is a North Carolina Medicaid program designed to help eligible adults remain in their homes rather than move into a nursing facility.

CAP Case Management is about more than paperwork or checklists. It’s about listening, advocating, and planning care that fits real life.

Our case managers work closely with individuals, families, healthcare providers, and community partners to create personalized care plans that support health, safety, and independence. We don’t take over—we help coordinate and strengthen the supports already in place.

Who We Serve

CAP may be appropriate for adults who:

  • Have a disability or chronic condition
  • Need assistance managing safely at home
  • Qualify for Medicaid
  • Are at risk of nursing facility placement

You don’t need to have all the answers before reaching out. One of the most important parts of our role is helping individuals and families understand whether CAP may be a good fit and what next steps look like.

The Role of a CAP Case Manager

Our case managers are boots-on-the-ground advocates. They:

  • Visit individuals in their homes
  • Complete thorough assessments
  • Coordinate needed services
  • Monitor changes over time
  • Support caregivers and families
  • Adjust care plans as needs evolve

Life changes—and good care should change with it.

Supporting Families and Caregivers

Caregivers often carry more than they let on. CAP Case Management helps reduce caregiver stress by providing guidance, coordination, and reassurance. We help families understand their options and prevent small challenges from becoming major crises.

Supporting the caregiver is just as important as supporting the individual receiving care.
